An accompanying document shall be drawn up in respect of each batch of eggs for hatching or chicks dispatched and shall bear at least the following particulars:
(a) the name or business name and the address of the establishment and its distinguishing number;
(b) the number of eggs for hatching or chicks according to species, category and type of poultry;
(c) the date of dispatch;
(d) the name and address of the consignee.
