(1) Rehabilitatsiooniplaan on õigustatud isiku ja tema seadusliku esindaja osalemisel rehabilitatsioonimeeskonna koostatud dokument, milles kajastub hinnang isiku tegevusvõimele, sotsiaalse rehabilitatsiooni eesmärk, eesmärgi saavutamiseks vajalikud rehabiliteerivad teenused ja hinnang sotsiaalse rehabilitatsiooni eesmärgi saavutamise kohta.
