Tüübihindamise läbinud toote kohta annab litsentsitud asutus tarnijale välja lisa nr. 5 vormi kohaselt vormistatud tüübihindamistõendi, saates selle koopia 5 tööpäeva jooksul Tööinspektsioonile. Tõendile lisatakse toote identifitseerimiseks vajalikud kirjeldused ja joonised. Tüübihindamistõend on kehtiv kuni 5 aastat.
