(1) Surma põhjuse teatis on surnud isiku surmasündmuse, surma põhjuse, surma põhjuse tinginud asjaolu ja surma põhjuse tuvastanud isiku andmeid kajastav elektrooniline dokument, mille alusel kantakse andmed surma põhjuste registrisse.
